Thank you for choosing Main Street Dentistry! We can’t wait to see you for your first appointment. Here are a few things to keep in mind before your initial visit to make the process as smooth as possible.

Patient Registration

To save time on the day of your appointment, you can pre-register with our office by filling out our secure Patient Registration Form. Your privacy is important to us, so please know that we’ll never share your information with any third parties. If you’d prefer to register in person, please arrive 15 minutes before your appointment to fill out the necessary paperwork.

Initial Appointment

In order to set up your patient profile and guarantee the best service, you’ll need to bring a few things to your first appointment. We’ll need to see:

• Proof of insurance, along with any forms you’ll need in order to file a claim
• Any x-rays taken by previous dentists, if applicable to your reason for visiting (see “X-Rays” section below)
• A list of any medications you’re currently taking

During your first appointment, we’ll go over your dental history and give you a complete examination. From there, we can discuss any treatment options you might need. In most cases, the treatment can be completed on the same day. However, if any issues arise or if you have a complicated medical history, we reserve the right to schedule your treatment on a different day.

X-Rays

If you’ve had x-rays taken by a previous dentist, you can call their office and ask that they forward them to our office. If you need additional x-rays taken, we can do that at our facility.
